When Getting to the Doctor Isn't Easy

For some patients, getting to a medical appointment requires much more than simply getting into a car.

Wheelchairs, walkers, medical transportation, physical limitations, and dependence on caregivers can make even routine medical visits difficult.

For patients who are homebound or have limited mobility, a trip to the emergency department can be physically and emotionally exhausting.

We understand these challenges and believe that access to quality medical care shouldn't depend entirely on a patient's ability to travel.

Medical Care That Meets You Where You Are

Many medical concerns can be evaluated through a scheduled telephone or video consultation. When clinically appropriate, our providers can recommend treatment, prescribe medications, and help determine the next steps.

Medication Management

Review medications, address possible side effects, reconcile medication lists, evaluate prescription refills, and discuss appropriate dosage adjustments.

Acute Illnesses

Evaluate selected respiratory symptoms, urinary concerns, gastrointestinal symptoms, and other common illnesses when telemedicine is appropriate.

Chronic Condition Follow-Up

Discuss selected stable chronic conditions, review home monitoring information, and evaluate concerns that may require treatment adjustments or additional testing.

Skin and Wound Concerns

Review photographs or video of rashes, skin irritation, and selected wound concerns, while recognizing when hands-on examination is necessary.

Post-Hospital Follow-Up

Review discharge instructions, medications, available test results, and ongoing recovery concerns.

Caregiver Guidance

Help family members and caregivers understand medical concerns, treatment recommendations, and warning signs requiring additional care.

Sometimes There's a Middle Ground

For families caring for an elderly or homebound loved one, medical decisions can sometimes feel overwhelming.

When a new medical concern develops, it may seem like the only choices are a trip to the emergency department or waiting and hoping things improve.

But not every situation requires hospitalization.

In selected circumstances, there may be reasonable treatment options that can be evaluated or managed at home. This might include medication review, appropriate prescription treatment, symptom monitoring, or arranging follow-up testing when available.

Our goal is to help patients and families understand their options, respect their healthcare preferences, and determine the safest and most appropriate next steps.

When emergency or in-person care is necessary, we'll explain why.

Helping patients receive the right level of care, in the right setting, whenever possible.

Support for the Families Who Care for Them

Caring for an aging parent, family member with a disability, or loved one with complex medical needs can be challenging.

Sometimes you're trying to manage medications, understand new symptoms, coordinate appointments, and make difficult healthcare decisions.

Our telemedicine visits can help patients and caregivers better understand medical concerns and develop appropriate treatment plans.

With the patient's permission or appropriate legal authorization, family members and caregivers can participate in telephone or video appointments.

How Our Care Works

  1. Schedule a Visit

Book a convenient telephone or video consultation using insurance or cash pay.

  2. Discuss Your Concerns

Meet with an experienced medical provider to review symptoms, medications, medical history, and available home monitoring information.

  3. Develop a Plan

When appropriate, receive treatment recommendations, prescriptions, follow-up instructions, or guidance about additional testing and in-person care.

When Telemedicine Isn't Enough

While many medical concerns can be evaluated through telemedicine, some conditions require an in-person examination, emergency department evaluation, or hospital-level care.

Patients experiencing severe breathing difficulties, chest pain, stroke-like symptoms, new significant confusion, serious injuries, uncontrolled bleeding, or rapidly worsening symptoms should seek emergency medical care.

Telemedicine cannot replace hands-on examinations, emergency monitoring, imaging, IV treatment, or other services that require an in-person healthcare setting.

Our providers will recommend an appropriate level of care based on the patient's symptoms, medical history, and clinical circumstances.

Senior Healthcare Is a Continuum: Finding the Right Balance Between Treatment and Quality of Life

As we age, our healthcare needs and priorities often change. There may come a time when patients and families decide that aggressive medical interventions, repeated hospitalizations, or certain preventive screenings are no longer consistent with their goals.

But choosing a less aggressive approach doesn't mean choosing to go without medical care. There is often a middle ground between pursuing every possible medical intervention and transitioning to hospice care.

Patients may still want infections treated, medications adjusted, symptoms managed, and medical support provided at home whenever safely possible. In this educational video, we discuss how healthcare decisions evolve with age, the importance of individualized treatment plans, and how patients and families can work with their medical providers to prioritize comfort, independence, and quality of life.

We accept most major insurance plans, including Medicare, Priority Partners, BlueCross BlueShield, UnitedHealthcare, Aetna, Cigna, Humana, and Tricare.
